A therapist and case manager will collaborate with each client to help develop a personalized discharge plan, including securing a stable living situation prior to discharge. They will schedule an appointment for each client’s next phase of treatment to ensure ongoing success in recovery. On discharge day, treatment team members will assess clients for stability, and Cedar Crest willprovide transportation home as necessary.
