Chatham Oaks Residential Care Facility (RCF) provides residential care for adults seeking support for chronic mental illness. This facility provides a 24-hour well monitored, structured and supportive environment for their patients. It offers individualized, person-centered service plans that help residents develop skills for greater independence and community living. The center’s approach focuses on skills-building to help residents live independently. The residential program provides various services to attend the needs of the patients. It has its own nursing services, care coordinators, social work support, and organized recreational activities to help clients to confidently manage their conditions and learn the skills needed for reintegration to the community. Chatham Oaks offers daily and hourly home-based habilitation services for individuals with a primary mental health diagnosis. The program prepares individuals to fully integrate into the community through 24-hour support based on individual needs, empowering them to manage their conditions, live independently, and develop life skills.
