Their children's case management services are for individuals under 18 years old who have a form of Minnesota Medicaid (MinnesotaCare). HDC offers ongoing monitoring and care coordination and will help connect families to necessary community resources. They will provide advocacy and support as appropriate. Through their Project Reach Out program, HDC offers support and mediation for youth and their families. They host many activities and events for youth like art nights and movie nights.
