The behavioral health outpatient program in San Jose serves adults who are living in the community or in supportive transitional housing. Each client receives an individualized care plan after a thorough assessment from a member of the team. The clinicians take a whole-person approach to treatment which includes mental, emotional, physical and social well-being. They offer diagnoses, employment services, individual or group therapy, psychiatric care, skill development and symptom/medication management. Clients also have access to a case manager who can help navigate the services clients may qualify for.
