Pathways Rowan County Horizon Village

Located near Morehead, Kentucky, the center serves adults with intellectual and developmental disabilities (IDD) in a supportive residential setting and provides on-site outpatient care. The center promotes independence, dignity, and empowerment through evidence-based behavioral health and substance use services. It treats co-occurring mental health and addiction issues with care levels ranging from supportive housing to intensive outpatient programs.
Horizon Village delivers a broad suite of outpatient therapies including individual, group, dual diagnosis, family counseling, assertive community treatment (ACT), and intensive outpatient programs (IOP). They tailor care using evidence-based interventions, peer support, telehealth, trauma-informed methods, and medication-assisted treatment (MAT), ensuring a holistic and personalized approach to mental health and recovery.
Residents live in one of 15 private apartments or in a 15-bed shared unit, providing personal living space with access to shared common areas. The site offers meals, laundry, and community activities designed to build life skills and foster social engagement. It balances independence with communal support to encourage full participation in daily living.

Evidence-based therapies including 1-on-1 Counseling and Cognitive Behavioral Therapy, typically 30 days.

Regular progress reviews, skill-building sessions, and adjustment of your treatment plan.

4. Aftercare Planning

Continued support, relapse prevention strategies, and connections to community resources for lasting recovery.
