Social Development Commission Counseling and Wellness Clinic

This clinic offers outpatient care for adults, families, youth, older adults, women with children, and justice-involved individuals facing substance use or mental health challenges. Services include individual, group, and family counseling, as well as case management, referrals, consultations, and advocacy. Care is provided at no cost to low-income residents through an agency committed to reducing poverty and supporting vulnerable communities in Milwaukee County.
Each visit centers on listening, setting goals, and developing skills for recovery and well-being. Clients may take part in individual counseling, group sessions, or family meetings that strengthen communication and support at home. Case managers also connect individuals with community programs and services to extend progress beyond the clinic. Every session is structured to promote stability, stronger relationships, and lasting growth.

1. Initial Assessment

A clinical team evaluates your history, needs, and goals to create a personalized plan.

2. Active Treatment

Evidence-based therapies including 1-on-1 Counseling and Cognitive Behavioral Therapy, typically 30 days.

3. Progress & Recovery

Regular progress reviews, skill-building sessions, and adjustment of your treatment plan.

4. Aftercare Planning

Continued support, relapse prevention strategies, and connections to community resources for lasting recovery.
