Located in Jacksonville, this 150-bed residential reentry center is part of The Transition House’s recovery network, supporting adult men exiting incarceration, homelessness, or substance use. The program combines reentry planning, substance use recovery, mental health care, and employment readiness. Residents participate in full-time community work, meet legal obligations like court and family payments, and receive support rooted in dignity, structure, and personal responsibility.
Residents follow a structured routine grounded in evidence-based practices. Services include therapy, trauma-informed care, medication management, and coaching. Educational offerings include basic education to finish high school, trade certifications, and adult learning. Employment support includes job readiness and the Men’s Warehouse Re-entry Project, which provides professional clothing to build confidence. Activities such as Alcoholics Anonymous (AA), Narcotics Anonymous (NA), and peer mentoring foster emotional healing and help residents build the skills and support needed for lasting recovery and independence.
The full-service facility provides 24/7 care, healthy meals, case management, and financial education. Veterans may access additional housing and employment resources through the VA’s Supportive Services for Veteran Families (SSVF) program, which helps low-income veteran households achieve housing stability. Residents work toward permanent housing and continued progress through discharge planning. Inspire Counseling offers outpatient services for those who’ve completed the program, supporting lasting wellness beyond residential care.
